Job Description Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by Home Health Manager through patient record documentation, verbal communication and direct observation.

Required Qualifications: - Master's Degree: MSW from a school of Social Work.
- Within 30 days of hire: National Provider Identifier (NPI) number
- 1 year experience in a health care setting.
- Upon hire: Washington Advanced Social Worker License Or
- Upon hire: Washington Advanced Social Worker Associate License Or
- Upon hire: Washington Clinical Independent Social Worker License Or
- Upon hire: Washington Clinical Independent Social Worker Associate License
- 3 years Post Master's work experience, or prior experience in healthcare care setting.

Preferred Qualifications: - 2 years experience in a health care setting.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
