Location: Orange County

Responsibilities:

• Assesses the environment of the home and community to identify elements which impact negatively on the patient/family ability to function.
• Provides direct short-term counseling services to patient/family or makes appropriate community referrals.
• Teaches coping skills to patients/families to assist them in their adjustment to the life changes associated with illness/disability.?

• Participates in conferencing and planning with other disciplines caring for the patient.?
• Prepares clinical progress notes and documents in the patients record, as indicated .?
• Available for planned social work consultation on patient care with staff, as needed.?
• Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.

Job Type: Fee for Service

Salary: $70 per visit

Qualifications:

• New York State Social Work License
• 2 years of social work experience

Education:

• Masters Degree
