Job Summary: The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time TH Staff Assistant 2 / Patient Advocate, Mother Cabrini Health Foundation. The successful candidate will:
- Be responsible for screening patients.
- Conduct social determinant of Health (SOOH) screenings for inpatients and supporting the patients
- Help patients through the screening and follow-up of a medical/psychosocial conditions.
- Help patients set up appointments, and provide financial, legal, and social support/referrals and communicating with healthcare providers.
- Assist patients communicate with their healthcare providers so they can get the information they need to make decisions about their care.
- Identify barriers to care such as logistics, transportation, or language barriers etc.
- Gather and document patient issues, barriers, or concerns. May also need to provide reports for management review.
Required Qualifications: - High School Diploma.
- Excellent communication and interpersonal skills.
- Empathetic attitude, with an ability to offer emotional support.
- Passion for the proper care and well-being of patients.
Preferred Qualifications: - Bachelor's Degree in a healthcare-related field preferred.
- 2+ years experience in a healthcare setting preferred.
- Knowledge of healthcare systems, insurance plans, and patient resources.
Work Schedule: Monday to Friday; 9:00am to 5:00pm (Full-Time)
Salary Grade/Rank: SL-1
S alary Range: Commensurate with experience and qualifications
