What You’ll Do: Deliver direct care to hospice patients in their homes, supporting their comfort and dignity under the supervision of a Registered Nurse. Assist with daily living activities, hygiene, mobility, and vital sign monitoring. Provide specialized care such as catheter and ostomy support, dressing changes, and equipment use. Observe and report changes in patient conditions and follow the established care plan. Serve as a compassionate presence to patients and a supportive resource to families. Promote safety and adhere to Elara Caring’s high standards for infection control, HIPAA, and professionalism. Participate in ongoing training and education to enhance your skills and stay compliant.
